I think I may have narrowed down the issue. I always go into advance  settings and use custom settings for output. 1920x1080 60fps H264 at a bitrate of 30mbit/s and 

It's off every time. I reset to 

HD 1080p and finally got a video to sync up. This doesn't resolve the issue because it does not allow me to use 60fps and higher bitrates.  It ruins my picture quality. 1920x1080p @ 30fps and 12000 kbit/s isn't  enough frames or kbit/s to keep the picture quality up.
